故事開始於一座名為克利得摩的監獄之中，在很多年前，這座監獄裏發生了一些非常可怕的事情，導致監獄被關閉了。一晃眼多年過去，漸漸淡忘了從前悲劇的人們決定重新開啟監獄，霍頓（阿倫·迪安·斯奈德 Arlen Dean Snyder 飾）被派往了這裏，成為了監獄的典獄長。實際上，這並不是霍頓第一次來到克利得摩了，他曾經是這裏的警衛。
當監獄重新開放之後，這裏關押的犯人和在這裏工作的職員們漸漸發現，在這所監獄裏，正在發生一些科學無法解釋的邪惡的事情，尤其是犯人們，他們遭受了非人的折磨。為了拯救這些無辜的人，犯人康尼（維果·莫騰森 Viggo Mortensen 飾）和犯罪學家凱瑟琳（Chelsea Field 飾）決定聯手，調查多年前事件的真相，阻止悲劇的重演。
